Scholarship Referee Form 2026

1. Complete this form only if you have either received an admission offer or are currently attending a post-secondary program at a post-secondary institution in Canada.
2. Applicants must be living in Canada and be able to attend a virtual interview.
3. Two or three completed recommendation/referee forms supporting your application are essential.
4. References can be from teachers/employers/community leaders/organization leaders.
5. Please complete this application once, with as much information as possible.
6. You can attach additional documents that support your application. (optional)
7. You can also attach a short video clip in support of your application. (optional)

9. Application package must be submitted before the deadline date.
10. All applications will be carefully reviewed. Incomplete applications will not be considered.
11. Respond to all the questions on the application form and provide as many details as you can. Use as much space as you need.
12. All decisions are Final.

1. The completed form: Section B.
2. A clear Photograph (head and shoulders) in JPEG format.
3. A copy of the Letter of Acceptance from the educational institution and
4. A copy of your current Transcript.
5. The Consent Form.
6. Optional: Any other documents or short video clips can be added in support of your application
7. References should be sent directly from the referee

References: Referees should complete the Reference form. Two or More are required from teachers/employers/community leaders /organizations’ leaders. Referee letters or completed forms must be sent directly from your referee to [email protected]

Submission Details: Completed application packages should be e-mailed to [email protected] by the deadline date of March 22nd, 2026
